What a pro does

Slab leaks are less common around Omaha than in slab-on-grade parts of the country, since most houses here have a full basement. Where they turn up is in the newer additions, garage floors, walkout lower levels, and the ranch builds out toward Papillion and Bellevue where a portion of the floor sits on poured concrete. The work starts with confirmation, not excavation. A plumber isolates the system and watches for pressure loss to establish that there's a leak at all, then uses acoustic listening gear to narrow where it is under the slab. Line tracing equipment maps the run so the search area shrinks to a few feet instead of a room. Only then does anyone talk about opening concrete. Depending on where it lands, rerouting the line overhead or through an accessible wall can beat breaking the floor, and in a finished lower level that trade is worth taking seriously. If the slab does get opened, the repair gets pressure tested before the patch goes back down, because nobody wants to cut the same floor twice.

Concrete and the gravel under it will absorb and carry water sideways for a long time before anything appears at the surface. Rising pressure loss and the sound of movement with the house quiet are usually the first honest signs.
Most of the older stock is, which is why this doesn't come up as often as it does further south. Garages, additions built onto the back of a house, and some newer ranch layouts on the western edge of the metro do have pipe running in or under concrete.
It's a contributor rather than a cause. Scale narrows the pipe and raises velocity inside it, and copper that's already thinned at a bend wears through faster where the water moves hardest. Soil chemistry and how the line was installed matter at least as much.
It depends on the layout. A single failure on an accessible run is often simplest to repair in place. If the same line has already leaked more than once, abandoning it and running new pipe through reachable space usually ends the problem for good.
